A custom oral appliance fabricated from a patient-specific dental impression to advance the lower jaw during sleep.
This custom mandibular advancement device (MAD) is a patient-specific oral appliance used in clinician-directed management of obstructive sleep apnea and snoring when oral appliance therapy is appropriate. The device is fabricated from an individual dental impression and positions the lower jaw forward during sleep, which can help maintain upper-airway space.
Two ordering workflows are available: Digital Impression for cases submitted from an approved intraoral scan, and PVS Impression for traditional physical impressions. Because a custom sleep appliance affects the teeth, jaw position, bite, and temporomandibular structures, it requires professional evaluation, accurate records, fitting, titration, and follow-up. It is not a direct-to-consumer mouthguard. Clinical suitability, prescription requirements, expected outcomes, and monitoring should be determined by the treating sleep and dental professionals.
Return requests must be submitted within 14 days of delivery and approved before anything is sent back. Eligible items must be unopened, unused, complete, and factory sealed where applicable. Opened medical devices, oral-contact items, nasal devices, tubing, and other hygiene-sensitive products are final sale. Authorized non-defective returns may be subject to a 20% restocking fee and return shipping costs. Report damaged or defective items within 48 hours of delivery.
