Appointment Cancellation, No-Show and Billing Policy

Rules for scheduled clinical services, cancellations, missed appointments, technology failures, charges, and billing questions.

Scheduling and Cancellation

Please cancel or reschedule a scheduled clinical appointment at least 24 hours before the appointment when reasonably possible. You may use the scheduling functions made available to you or contact support@sleepbalanceacademy.com. A late cancellation or missed appointment may be subject to a fee only if the amount and circumstances were disclosed before the appointment, the fee is permitted by applicable law, and no applicable contract or program rule prohibits it. We may waive a fee when illness, emergency, caregiving, technology failure, or another circumstance outside the patient's reasonable control caused the missed appointment.

Late Arrival

If you are late, join or contact us as soon as possible. The clinician may continue the appointment if there is enough time to provide the service safely and appropriately. Otherwise, the appointment may be rescheduled. A shortened appointment will not be used when doing so would compromise care.

If SBA or the Clinician Cancels

If SBA or the clinician cancels an appointment, you will not be charged for a service that was not provided. We will make reasonable efforts to offer a replacement appointment or another appropriate arrangement. If you prepaid for a service that is not provided and is not rescheduled, we will refund the applicable prepaid amount.

Technology Failure

If a telehealth appointment cannot be completed because the technology is inadequate, the clinician may attempt an alternative legally permitted method or reschedule. You will not be charged as though a full service was delivered when the clinical service could not be provided. If a substantial portion of the service was completed, any charge or follow-up arrangement will reflect the service actually furnished and applicable law.

Clinical Fees and Outcomes

We disclose the price or billing basis for self-pay clinical services before scheduling or before payment is taken. A clinical fee pays for professional time, evaluation, judgment, documentation, and clinically appropriate follow-up associated with the service. It does not purchase or guarantee a particular diagnosis, prescription, device, treatment plan, test interpretation, or outcome.

Insurance and Self-Pay

If insurance is used, coverage, cost-sharing, prior authorization, and network status may depend on the health plan and are not guaranteed by an estimate. If you are uninsured or choose not to use insurance, we will provide a Good Faith Estimate of expected charges when required by the No Surprises Act and implementing rules.

Refunds for Clinical Services

A prepaid clinical service that is canceled in accordance with this Policy before it is furnished will be refunded as applicable. A completed clinical service is not refundable merely because the clinical conclusion or treatment recommendation was different from what a patient hoped to receive. Nothing in this section limits a refund or remedy required by law or an obligation to correct an erroneous charge.

Billing Disputes and Records

If you believe a charge is incorrect, contact support@sleepbalanceacademy.com and identify the charge. We will review the account and correct an error. A billing dispute does not reduce your rights under the Notice of Privacy Practices, including your right to request access to your health information. SBA may use lawful billing, collection, scheduling, or payment-arrangement practices for amounts properly due, but will not retaliate against a patient for making a good-faith billing or regulatory complaint.
