Practitioners must select the appropriate Service, Location, and Device configuration below to ensure compliance with state licensure and insurance requirements.

Services Offered:

  • Insurance Telemed (Tx Rx Included): A strictly regulated protocol for Medicare/Insurance patients. This service includes two consultations: an initial consult to establish medical necessity before the sleep study is dispensed, and a follow-up to review results. Crucial: Do not dispense the HST prior to the first consult.

  • Non-Insurance Telemed: A streamlined protocol for self-pay patients. This service allows the practitioner to dispense the office-provided Home Sleep Test (HST) immediately, prior to the physician consultation. Includes one comprehensive review consultation.

  • Mail-Order HST (Insurance Coverage): For offices without device inventory who wish to utilize insurance benefits. Includes the mandatory pre-study consultation to ensure insurance compliance, mail the device directly to the patient (Round-trip shipping included), and perform the follow-up review.

  • Mail-Order HST (Non-Insurance): For offices without device inventory and Self-Pay patients. We mail the device directly to the patient for testing, followed by a consultation for interpretation and treatment prescription. Round-trip shipping included.

  • Consultation: Standalone services for ongoing patient management. Select "Re-Interpretation" for titration progress checks (post-treatment adjustment), "Follow-Up Consult" for general medical inquiries without new data, or "Bongo Rx Consult" for specific device prescriptions.

Service Guidelines

This service is for practitioners who dispense their own Home Sleep Test (HST) devices from their office but require a physician's oversight to comply with Medicare and private insurance reimbursement rules.

Strict Prerequisite: To comply with insurance guidelines, a specific order of operations must be followed. You cannot perform the sleep study on the patient BEFORE the initial telemedicine consult.
  1. Purchase: Select "Insurance Telemed" and the correct Location.
  2. Screening: Issue the SBA Screening Form to the patient. Do not issue the sleep study yet.
  3. Consult #1: Schedule the initial telemedicine consult with Sleep Medicine Speacialist. He will review the screening and issue a Prescription for the sleep study.
  4. Dispense: Once the Rx is issued, dispense your office's HST to the patient.
  5. Consult #2: After the test is complete, upload the data and schedule the second consult.
  6. Finalize: The physician reviews the data, provides a diagnosis, and issues the LMN and Prescription for treatment (if appropriate).

A streamlined service for practitioners dispensing their own HST devices to patients who are paying out-of-pocket. This option bypasses the initial pre-study consultation for faster diagnosis.

Prerequisite: This path is not suitable for patients submitting claims to Medicare.
  1. Purchase: Select "Non-Insurance Telemed" and the correct Location.
  2. Dispense: Issue your office's HST to the patient immediately (we prefer a minimum of two nights of data).
  3. Submit: Send the patient screening form and HST recordings.
  4. Consult: Schedule the single Telemedicine Consult.
  5. Finalize: Sleep Medicine Speacialist will perform the consult, interpret the study, and provide the LMN and Prescription for treatment in a single step.

Re-Interpretation Consult:

  • Purpose: To check the progress of treatment (CPAP or Oral Appliance) after titration.
  • Requirement: You must submit a final "endpoint" HST showing stats with the treatment device.
  • Outcome: Evaluates progress to determine if therapy is effective; often needed for final insurance approval.

Follow-Up Consult (No Interpretation):

  • Purpose: For patients who already have an interpreted sleep study but need a medical consult to secure a Letter of Medical Necessity (LMN) or updated Rx.
  • Note: This does not include the interpretation of raw sleep study data.

Bongo Rx Consult:

  • Purpose: A focused short consult specifically to authorize a prescription for the Bongo Rx EPAP device.
